High School Summer Sports Programs

Required

The High School Summer Sports Programs are designed for incoming 9th Grade, transferring students in Grades 10th - 12th, and returning students. These programs will offer you a great chance to develop your skills and learn the importance of team play while meeting your future coaches and teammates.
 
Parents, this registration form is for one student but can be used for multiple sports/programs. Please register separately for additional students.
 
Please note: a credit card payment convenience fee of 3.1% has been included in all of the costs listed on this form. If you prefer to pay by another method, please contact Mr. Mike Van Briggle at mvanbriggle@louisvillehs.org.

I hereby authorize the staff of the Louisville Sports Programs to act for me according to their best judgment in any emergency requiring medical attention for my child, and I hereby waive and release the Programs from any and all liability for any injuries or illness incurred while my child is at the Programs.
